Employees (30 or more hours) are eligible for health insurance coverage. Health coverage will be effective on the first day of the month following the employee's start date. Dental Insurance — Employees (30 or more hours) are eligible for dental insurance. Pines pays 100% of the plan cost for the employee. The employee pays the cost of family coverage. For details about dental coverage, click here: Dental Insurance Detail.
All full-time employees (35 or more hours) are eligible for vision coverage. Pines Health Services self-administers a vision plan. Pines pays for the cost of employee coverage and the employee pays family coverage. Pines provides $300.00 of benefit coverage to be used for eye exams, lenses, frames and contacts. Employees are responsible for paying expenses, and submitting original receipts for reimbursement. Reimbursement will be made after all insurances have been paid. Coverage is effective on the first day of the month following the start date.
All employees are eligible to participate in retirement options. Pines Health Services has chosen Lincoln Financial Group to provide investment options and administrative services for our 403(b) retirement plan. All full-time employees (35 or more hours) are eligible for life insurance. Pines Health Services provides Basic Life and Accidental Death and Dismemberment (AD&D) insurance to employees in an amount equal their annual earnings to a maximum of $100,000.00. This coverage is provided through Lincoln Financial. If you would like additional life insurance to meet your financial needs, you can purchase voluntary life insurance through after-tax payroll deductions. Coverage elected will be in increments of $10,000.00 to a maximum of $200,000.00 or five times annual earnings. Dependent life insurance is also available. This coverage provides a spousal benefit of $10,000.00 and $6,000.00 for dependent children.
All full-time employees (35 or more hours) are eligible for Short Term Disability Insurance. Pines Health Services offers Short Term Disability through Lincoln Financial. Employees are responsible for the premium if coverage is elected. The cost of this benefit is based on your annual salary. The short-term disability plan pays a benefit for each week you are unable to work because of injury or illness. Your weekly benefit is 60 percent of your pre-disability earnings reduced by deductible income. All full-time employees (35 or more hours) are eligible for Long Term Disability Insurance. Pines Health Services offers Long Term Disability through Lincoln Financial. The long term disability plan pays a benefit for each month you are unable to work due to a disabling condition. Enrollment is automatic and Pines pays the full cost for your coverage. Your monthly benefit is 60 percent of your pre-disability earnings reduced by deductible income. Benefits begin on your 91st day of disability and continue until age 65 if you are unable to return to work. Important contract limitations apply. Full-time (35 to 40 hours) and part-time (16 to 34 hours) employees are eligible to accumulate Earned Benefit Time (EBT). Pines Health Services offers an EBT program for vacation, holidays, and sick time. This means that for every hour worked, you accrue a portion of that time as EBT. The basic annual accrual for a regular full-time employee is 15 days for the first year. Upon completion of a 90-day probationary period, an employee may use accrued time as s/he chooses, providing that staffing needs have been met. Employees have the ability to carry over EBT from year to year up to a maximum accrual of 40 days or 320 hours. At that point, EBT stops accruing, as it is important for our employees to periodically spend time away from their job.
NOTE: Providers' EBT is based on contract.
